Community Health Scholarships
In an effort to support meaningful social transformation, Third Root is committed to addressing equality on all levels. Clearly, equality in a sliding scale fee structure that strives to make our health care services affordable across all income levels. However, we recognize there is still a population of those for whom even the low end of that scale is too much to negotiate into their tight budgets.
In an effort to bridge that gap in health care access and build community across class divisions, Third Root has also developed a Community Health Scholarship Incentive program. This program encourages those who have a little or a lot more to consider purchasing scholarships. Half of the scholarship would go to your health care and the other half would support someone in the community who may not be able to afford our services. There is nothing better that knowing you make a difference. When each one helps one, we all become better and healthier.
Community Health Scholarships may be purchased at the $100, $500 and $1000 levels, with other sizes and generosities open to negotiation. Scholarships may be donated anonymously or with your name attached to the scholarship. The choice is yours.
How can I apply for a Community Health Scholarship?
We have an application form for you to fill out, which will ask some basic questions about your current health and health history, as well as your vision for your health — where do you want Third Root to take you? We also ask that you fill out our Sliding Scale Worksheet and return it with your application. We will get back to you within two weeks.
How do I give a Community Health Scholarship?
Call us at 718.940.9343, email scholarships@thirdroot.org, or talk to anyone at the front desk of the Third Root Community Health Center. We will give you a donor form to fill out and then we will move forward in health justice from there!
